Form Field Types

Basic Form Fields #

Basic Forms are limited to the following fields:

• Name
• Email
• Phone Number
• Short Text Field (10 available)
• Long Text Field (5 available)

Advanced Form Fields #

Advanced Forms can include an unlimited number of the following field types:

• Short Text: for a maximum 100 characters
• Text Area: for long form text responses
• Radio: for multiple-choice questions
• Checkbox: for yes/no questions
• Drop Down: for multiple-choice questions with more than 6 options
• Phone Number
• Email (required if you would like to send an automatic email to the responder)
• Date
• Label Only: for section titles, instructions, etc.
• Rating Scale: for ratings questions
