Add Content to Forms

The form builder section is where you manage the content on your form. To access this page, navigate to the Questions tab on your form.

Questions

Sunergo forms allow the following question fields to retrieve information from responders.

Short Answer

For short answers with a maximum of 100 characters.

Paragraph

For long text responses with no character limit.

Radio

For multiple-choice questions.

Checkbox

For yes/no questions.

For multiple-choice questions with many options.

Phone

For phone numbers.

Email

For emails. Can be used to send a copy of responses to responders.

Date

For dates.

Purchase

For collecting an amount of items a responder wants to purchase. There are three fields you can customize for your purchase questions:

  • Maximum Units - The maximum number of items the responder can select. If set to 1, the question becomes a checkbox. If set above 1, the question becomes a dropdown.
  • Cost per unit - The cost of each item.
  • Attached Payment Fund - The fund this question is tied to. Learn more about attaching funds to forms.

Example: If Cost per unit is $10, the responder selects 6 items, and the attached payment fund is “T-Shirts”, after the responder submits the form they will be redirected to the “T-Shirts” payment page with a prefilled amount of $60 ($10 x 6 items).

Rating

For ratings with an adjustable max rating between 1 and 10.

There are two additional customizations that can be done to rating questions:

  1. The Left Label controls text that can show up on the far left of the rating scale. This can be used to indicate what the lowest rating represents such as “poor” or “disagree”.
  2. The Right Label controls text that can show up on the far right of the rating scale. This can be used to indicate what the highest rating represents such as “excellent” or “strongly agree”.

File Upload

For collecting files.

Supported file types:

  • .pdf
  • .doc
  • .docx
  • .txt
  • .jpeg
  • .png
  • .webp

Maximum supported file size: 5MB

Signature

For hand-drawn signatures. Signatures get saved as an image.

Action Questions

Action questions are fields that can trigger actions on your form. Learn more about actions.

Create Contact

The Create Contact question is a multi-part field used to obtain basic information of your responders. When filled out, a new contact record gets automatically added to your organization.

Furthermore, additional actions can be added alongside this question so new contacts created immediately get put into a ministry group, assigned a life event, and more.

Creating duplicate contacts are possible if information supplied to this question already exists. If you have Church Tools, this can be handled by using the Contact Merge Tool.

Presentation

Presentation items help you organize your form better and add additional information for your responders.

Section Heading

Used to separate sections on your form with large text.

Text

Provide supplementary information directly on your form.

Deleted Questions

After a form has received responses, questions can still be deleted. However, when questions that have answers from responders get deleted, they are moved into the Deleted Questions section for data integrity. The following is true once a question has moved to the Deleted Questions section:

  • The question is no longer available to fill out
  • Previous answers to this question are preserved and can still be viewed in responses
  • The deleted question can be restored at any time

Content on your form can conditionally appear based on the answers your responders select. To achieve this, content can be linked to the following question types:

  • Radio – Content can appear if a specific option is selected on a radio question
  • Checkbox – Content can appear if a checkbox is checked
  • Dropdown – Content can appear if a specific option is selected on a dropdown question

To link content to questions,

  1. Select the content you want to conditionally appear on the form editor.
  2. In the edit menu, open the Linked Question dropdown menu and select the question you want the content linked to. Ensure the linked question is above the content you are editing on your form.
  3. Open the Show Question if this option is selected dropdown menu and select the answer you want to link to this content.
